Student Health
Services
The
Student Health Office is located on the lower level of
Chatlos-Davis Hall. The Office of Student Development and
the college nurse administer the health program. Student
Health Services provides first aid, care for general
sickness, ordinary over the counter medicines and referral
to local physicians. Davis College students have two
excellent hospitals within two miles of the campus in case
of emergencies. The community also has many quality walk-in
clinics for non-emergency medical needs.
Davis College requires
completion of the following health forms (download forms at
left) for students
registered for 6 credits or more, on campus, at an extension
site, *independent study, *online or any combination of the
above totaling 6 credits or more. (*Certain exceptions may
apply, contact Student Health):

Personal Medical Report
completed by the student

Physician's Evaluation
(physical) dated within one year prior to coming
to campus

Immunization Record signed
by the Health Care Provider
-
Meningococcal Meningitis
Response Form (Freshmen students residing in the dorm
and high school students taking six credits or more on
campus MUST have the vaccine.)
-
Emergency Medical Consent
for students under 18 years of age
-
Insurance Enrollment/Waiver Form
declaring how the student is covered for medical
insurance. The College does offer a plan for those not
insured otherwise.

site, or, students may request a packet to be mailed
directly from the Student Health office (see contact
information below).
All form must be completed
and
returned to Student Health for review PRIOR
to coming to campus. All health records are kept in the
health office in professional confidence. Because of
confidentiality laws, faxed records are not accepted from
International Students and are discouraged from US
students.
In keeping with New York State
Public Health Law Section 2165, all students born
after January 1, 1957 taking 6 credits or more, MUST
show proof of having 2 doses of LIVE Measles,
Mumps & Rubella vaccine, the first being given no more
than 4 days prior to the child's first birthday and a second
dose no less than 28 days after the first dose. The law does
provide for a religious & medical exemption under certain
circumstances Contact Student Health for details. New
York State also requires the Meningococcal Meningitis
Response Form to be on file & no student is allowed to
attend classes or stay on campus beyond 30 days without
these 2 requirements on file.
