Access Form Page

The College network requires that everyone fills out an "Access Form" so that the following may be set up for you.

Username: Your username will be your full name in this format: firstnamelastname. This is used for your:

1. Email -- All information for your college experience will be sent to your college email address. No other email addresses will be used. You will receive information on how to connect and set up your email program.

2. Computer Center Access -- If you do not have a computer, the lab is where you can go to type papers and do other Internet related things.

3. Internet Access -- All Internet access is password protected and you must sign in.

4. Moodle online class site -- Some of our class materials are located on the campus education site. Your professor will inform you if you need to access this service.

5. Library Login -- Koha has separate logins for each use. You can place books on reserve through the Internet interface.

Password: The password you choose has to follow these guidelines:

1. No names or dictionary words.

2. Must contain both numbers and letters.

3. Must be at least 8 characters long.

4. The password is case sensitive and can use both upper and lower case letters.

5. Each person is responsible to keep their password secured. If you give your password to a friend to use, and they misuse the Internet access policy, you are held responsible for what happens. Please read the Internet Usage Policy for more details.
