Transcript Request Form

A permanent record of grades is maintained for each student by the Registrar. An official transcript may be obtained on request of the student subject to certain conditions as follows:

  • There is no charge for the very first transcript ever requested by a current or former student.
  • A fee of $5.00 will be assessed for each transcript requested thereafter.
  • No transcript will be issued unless the student's or former student's financial account is current.
  • All requests for transcripts must be in writing or electronic format and must include the student's social security number. (Written formats will also require a signature.)
  • Signed requests sent by FAX (607.797.5773) will be honored.
  • Transcripts are normally processed within five to ten working days of receipt of the fee in the Registrar's Office.
